      Usually what happens is that a design (or any piece of technical work) is reviewed by at least 2 people. The reviewers often suggest changes/corrections which have to be carried out before final acceptance. So, in a way, the first edition of a design usually gets ‘rejected’ because it’s rare to be completely correct the first time round!
